Social prescribing is an intentional and supportive pathway that connects people to non-medical supports in their communities to improve overall health and wellbeing. Through a personcentred and co-creative approach, it complements traditional medical care by addressing social determinants of health, such as isolation, food insecurity, or housing challenges, with tailored resources like exercise groups, arts programs, or peer support networks.
